Your Key Responsibilities:

As a Support Worker, you will play an important role in supporting participants to achieve their goals and live life on their terms. Your responsibilities will include:

• Supporting participants to work towards and achieve their personal goals
• Coordinating and supervising both individual and group activities
• Providing transportation to and from services, appointments, and community events
• Encouraging community engagement and building meaningful social connections
• Promoting independent decision-making, choice, and self-empowerment
• Delivering support in line with person-centred care principles and best practices
