TransCare is a multi-service provider with over 30 years of experience in the Upper Hunter. We have three core service areas in Transport, Community Wellbeing and In-Home Assistance.
The Transport section operates regular bus services within the Muswellbrook Shire Council and Upper Hunter Shire Council areas from Monday to Friday and a weekly service to Tamworth. A door- to-door service for direct transport is provided to clients that have been assessed by My Aged Care, clients who are on the National Disability Insurance Scheme (NDIS), or clients who are deemed transport disadvantaged. TransCare also has arrangements with Hunter New England Health and the Cancer Council to provide transport to medical facilities. We also provide monthly local and long distance bus trips as a social outing for clients.
The Community Wellbeing section provides community support services on an individual basis under the Commonwealth Home Support Program (CHSP). Services include home maintenance, meals, and social support in the form of visiting, companionship, and accompanied activities such as shopping.
We also offer a free Be Connected Program aimed at assisting older Australians to get the most out of going online.
The In-Home Assistance section operates under funding from CHSP which provides entry-level home support for older people who need assistance to live independently. We provide domestic assistance and personal care services and we are a registered provider for NDIS and Home Care Packages. We also have an in-house Regional Assessment Service (RAS) that provides assessment services.
